Job SummaryWe are seeking a skilled Physical Therapist to join our team, providing comprehensive physical therapy services to patients in need. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, while enjoying a rewarding and challenging career.
Responsibilities- Provide physical therapy services, including assessment, evaluation, and treatment planning, to patients in their homes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
- Update care plans regularly, as changes occur, and at recertification, to ensure optimal patient outcomes.
- Perform OASIS time point assessments and discharge planning process, adhering to Medicare criteria, policy, and procedure.
- Attend staff meetings, team conferences, and educational in-services, as required, to stay current with best practices and industry developments.
- A current, unencumbered State Physical Therapy License in the state of practice.
- A current driver's license, good driving record, proof of insurance, and reliable mode of transportation.
- One year of experience as a home care professional Physical Therapist, with competency in performing comprehensive assessments and OASIS-C.
- The ability to make sound professional clinical judgment, assess and document patient needs, and formulate individualized patient care plans.
- Proficiency in clinical skills, Microsoft Office, and the ability to drive in a car for 2-4 hours per day.
