Job SummaryWe are seeking a skilled Physical Therapist to join our team, providing physical therapy services to patients in their homes. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, while enjoying a rewarding and challenging career.
Responsibilities- Provide comprehensive physical therapy services, including assessment, evaluation, and treatment planning.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Supervise and instruct non-professional home health aide staff in the provision of personal care to patients.
- Update care plans regularly, ensuring that patients receive the highest level of care.
- Perform OASIS time point assessments and discharge planning process, adhering to Medicare criteria and policy.
- Current, unencumbered State Physical Therapy License.
- One year of experience as a home care professional Physical Therapist, with proficiency in clinical skills and Microsoft Office.
- Ability to make sound professional clinical judgment, assess and document patient needs, and formulate individualized patient care plans.
- Bonus potential of $2500+ per quarter.
