About the Role

At Healthcare in Action, we are seeking a highly skilled and compassionate Care Management Director to join our team. As a key member of our organization, you will play a vital role in enhancing the health and well-being of individuals experiencing homelessness.

Key Responsibilities
  • Team Leadership: Manage and support a team of Care Management professionals, providing guidance and coaching to ensure they meet performance targets and deliver high-quality care.
  • Care Coordination: Oversee the activation of eligible patients into CalAIM programs, driving team enrollment to meet or exceed targets.
  • Quality Assurance: Monitor direct reports' documentation and quality, ensuring compliance with health plan guidelines and evidence-based practices.
  • Program Development: Co-develop and approve individualized care plans with direct reports, utilizing clinical knowledge and evidence-based guidelines.
  • Stakeholder Engagement: Foster strong relationships with internal and external stakeholders, including health plans, hospitals, and community organizations.
  • Process Improvement: Identify barriers to work processes and lead efforts to improve efficiency and effectiveness.
Requirements
  • Education: Bachelor's or Master's degree in Social Work, Gerontology, or a related field.
  • Experience: 3+ years of people leadership experience, with some direct supervision experience, and 3-4 years of experience in homeless care management.
  • Skills: Strong interpersonal, critical thinking, and analytical skills, with excellent written and verbal communication skills.
  • CalAim Experience: Strong preference for experience with CalAim programs.
