We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at AccentCare, Inc. in Sacramento County, Placer County, and Eldorado County.

Job Summary

As a Hospice LPN, you will provide high-quality patient care and support to individuals and their families in their homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  • Participate in ongoing education and training to stay current with best practices and industry developments

Requirements

  • Graduate of an approved school of professional nursing
  • One year of experience as a Licensed Practical Nurse, preferred
  • Licensed to practice as a practical nurse or vocational nurse in the state of agency operation
  • Valid CPR certification
  • Ability to travel to all business locations
