Occupational Therapist Job Summary

We are seeking a skilled Occupational Therapist to join our team at Consonus Rehab. This is a rewarding opportunity to work with patients who require rehabilitation services to regain independence and quality of life.

About the Role: As an Occupational Therapist, you will be responsible for delivering personalized care plans that help patients achieve their therapy goals. You will utilize evidence-based clinical programs and pathways to ensure optimal patient outcomes.

Key Responsibilities:

  • Craft and implement individualized care plans tailored to each patient's needs and goals.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients.
  • Maximize function in physical, emotional, social, and cognitive realms for patients and families.

Requirements: A graduate from an accredited education program in occupational therapy, current state licensure, CPR certification, national provider identifier, and experience in post-acute care preferred.
