Position Overview

Job Summary

The Occupational Therapist will be responsible for assessing, planning, and implementing occupational therapy interventions to help patients achieve their maximum potential. This will involve conducting thorough evaluations, developing individualized treatment plans, and collaborating with other healthcare professionals to ensure seamless care.

Key Responsibilities
  • Conduct initial and ongoing assessments to evaluate patients' mobility, coordination, strength, and current level of function.
  • Develop and implement individualized treatment plans to address patients' occupational therapy needs.
  • Collaborate with interdisciplinary team members to ensure comprehensive care and optimal patient outcomes.
  • Document patient progress and outcomes in accordance with company policies and procedures.
  • Participate in discharge planning and education to ensure patients and their caregivers are equipped to manage their care post-discharge.
Requirements
  • Master's degree in Occupational Therapy from an accredited university/program.
  • Current, unrestricted state license as an Occupational Therapist in the state of practice.
  • Valid CPR certification and a valid driver's license.
Preferred Qualifications
  • Strong organizational and communication skills.
  • Attention to detail and ability to manage multiple priorities.
  • Proven experience in home health therapy and OASIS documentation.
