About the Role

Aequor Technologies is seeking a skilled Speech Language Pathologist to join our team in a skilled nursing facility setting. As a Speech Language Pathologist, you will be responsible for evaluating and assessing speech, language, cognitive-communication, and swallowing disorders in residents of skilled nursing facilities.

Key Responsibilities
  • Evaluate and assess speech, language, cognitive-communication, and swallowing disorders in residents of skilled nursing facilities.
  • Develop and implement individualized treatment plans to address communication and swallowing deficits, enhancing residents' quality of life.
  • Provide direct therapy services, including individual and group sessions, to improve speech, language, and swallowing functions.
  • Educate residents, families, and caregivers on strategies and techniques to support communication and swallowing needs at home and in the facility.
  • Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and physical therapists, to ensure comprehensive care for residents.
  • Conduct regular progress assessments, adjusting treatment plans as necessary to meet residents' evolving needs.
  • Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations and facility policies.
  • Participate in care planning meetings, providing expert input on residents' communication and swallowing needs.
  • Stay current with advancements in speech-language pathology, participating in continuing education and professional development opportunities.
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current state licensure or certification as a Speech-Language Pathologist (SLP).
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) is preferred.
  • A minimum of 1-2 years of experience in a skilled nursing facility or geriatric setting.
  • Strong knowledge of speech, language, and swallowing disorders, particularly in the elderly population.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Strong organizational skills and attention to detail.
  • Compassionate and patient-focused approach to care.
  • Proficiency in electronic health records (EHR) systems.
