RN Clinical Liaison

**Job Summary:**
As a RN Clinical Liaison, you will play a critical role in educating healthcare professionals about our hospice services and providing end-of-life solutions with clinical excellence, compassion, and dignity.
**Key Responsibilities:**
- Educate healthcare professionals about our hospice services
- Provide end-of-life solutions with clinical excellence, compassion, and dignity
- Collaborate with our team to ensure effective intake and referral processes
- Complete admissions processes for referred patients in emergency cases
- Provide accurate and timely information regarding our hospice services
- Serve as a patient advocate in evaluating and developing plans of care
**Requirements:**
- RN license in the state(s) of employment
- Minimum of 2 years of healthcare experience in hospice, home health, clinical liaison, or a related field
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment
