Habilitation Assistant Job Description

We are seeking a skilled and compassionate individual to join our team as a Habilitation Assistant. As a member of our team, you will work closely with our clients to develop and implement personalized care plans that meet their unique needs and goals.

Job Summary

The Habilitation Assistant will provide direct care and support to our clients, including assisting with daily living tasks, promoting independence, and maintaining a safe and healthy environment.

Responsibilities
  • Client Support: Provide direct care and support to our clients, including assistance with daily living tasks, meal preparation, and light housekeeping.
  • Behavioral Support: Assist our clients in managing challenging behaviors and emotions, while promoting dignity and respect.
  • Documentation: Accurately record client information, including progress notes, medication administration, and incident reports.

Requirements
  • Education: High school diploma or equivalent
