Mental Health Association - Western MA is seeking a dedicated Housing Support Coordinator. The Coordinator will engage in outreach, assessment, and connection with participants of the Community Support Program for individuals experiencing chronic homelessness.

Assist participants with a recent history of chronic homelessness in adapting to and sustaining their housing while linking them to essential community resources.

Carry out responsibilities with professionalism, adhering to MHA's Core Values (Respect, Integrity, and Compassion), policies, practices, program funding, and relevant regulatory guidelines.


Key Responsibilities:
Intake Coordination
  • Facilitate the necessary documentation for entry into MHA's CSP-CHI program, including but not limited to: verification of chronic homelessness, VI-SPDAT assessments, authorizations, policy notifications, and insurance details. Prepare initial Adult Comprehensive Assessments and treatment plans.
Service Planning
  • Collaborate as part of a multidisciplinary team to create Individualized Action Plans that leverage participant strengths to achieve their goals. Actively support the implementation of strategies outlined in the IAP and document progress as required.
  • Design, monitor, and execute program support strategies, crisis management, and behavioral plans in partnership with participants and the treatment team. Adjust support levels to meet the evolving needs of participants.
Housing Stability
  • Assist participants in securing safe housing and foster relationships with landlords.
  • Provide tenancy support by helping participants negotiate repairs and maintenance with landlords, ensuring compliance with housing regulations; guide participants in fulfilling tenant responsibilities such as upkeep, rental payments, and maintaining a drug-free environment.
  • Support participants in organizing home relocations, including utilities and address changes, and assist with packing and moving as necessary.
Income and Benefit Maximization
  • Support participants in securing and maintaining employment, training, or educational opportunities that lead to job placement.
  • Educate participants on accessing benefits and resources, including Social Security, EAEDC, SNAP/Food Stamps, and Fuel Assistance.
  • Empower participants towards independence through financial literacy training, budgeting, and assistance with banking and benefits management.
Community Engagement
  • Encourage participant involvement in community life by facilitating access to libraries, recovery groups, social activities, vocational programming, and local events.
  • Engage and empower participants to utilize community resources to enhance self-determination, rehabilitation, and recovery.
Health and Wellbeing
  • Assist participants in coordinating and attending medical, dental, and psychiatric appointments as needed. Monitor medication adherence and health issues in collaboration with the program's nursing staff.
  • Promote healthy habits and physical activity among participants.
  • Provide crisis support while on duty by responding promptly to urgent calls and keeping supervisory personnel informed of any situations.
Team Collaboration and Continuous Learning
  • Participate actively in team meetings and training sessions, maintaining required certifications and pursuing professional development opportunities.
  • Work collaboratively towards the shared vision of community-based flexible supports, demonstrating professionalism and effective communication.
  • Provide transportation to participants for essential appointments and engagements, ensuring vehicle readiness and compliance with agency training.
  • Report issues and incidents promptly to supervisory personnel and complete necessary documentation within required timelines.
  • Utilize online reporting systems and applications effectively, attending training to enhance computer skills as needed.
Safety Protocols
  • Promote a safe work environment by identifying safety hazards and reporting incidents as per MHA policies.
  • Document any incidents of participant abuse or neglect according to agency protocols.
Qualifications:
Knowledge, Skills, and Abilities:
  • Valid driver's license, adequate automobile insurance, and a reliable vehicle during work hours.
  • Ability to work independently, manage time effectively, and collaborate as part of a team.
  • Strong oral, written, and organizational skills.
  • Capacity to adapt to the changing needs of program participants.
  • Demonstrated interpersonal skills, patience, and compassion.
  • Must meet certification requirements in medication administration (MAP), CPR, and first aid.
  • Minimum age of twenty-one years.
Education and Experience:
  • Bachelor's degree in human services or a related field, or equivalent lived experience.
  • At least six months of relevant experience, preferably with individuals facing mental health or substance use challenges or housing instability.
