About the Job
We are seeking a skilled Case Manager to work with tenants at Belmont Daniel Supportive Housing in Staten Island. As a Case Manager, you will be responsible for conducting intakes, assessments, and re-assessments, as well as developing and maintaining tenant files. You will also collaborate with tenants to create and revise their independent living plans and identify community resources to support their goals.

Responsibilities
• Conduct regular home visits to assess tenants' living conditions
• Assist tenants in completing applications for benefits and entitlements, and process applications on their behalf
• Provide all required information for weekly/monthly/quarterly/annual reports
• Participate in case conferences and follow up with tenants regarding their progress
• May act as tenant liaison/tenant advocate with outside organizations regarding education, healthcare, housing, legal issues, etc.

Requirements
• Bachelor's degree (e.g., B.A., B.S.W.) and experience working with the Seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience
• Ability to work flexible hours occasionally to provide shift coverage until 8 pm

We offer a salary range of $40,000-$45,000 annually, plus a comprehensive benefits package, including health insurance, dental insurance, 403(b) retirement plan with employer match, paid time off (vacation, personal, and sick time), and paid holidays.
