Home Care LPN Opportunity

Join Interim HealthCare of the Twin Cities as a Licensed Practical Nurse (LPN) in our Home Care team. As an LPN, you will have the opportunity to work independently and autonomously, visiting patients in their home environment. This role offers a unique chance to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services ordered by the physician as indicated in the plan of care
  • Counsel and educate the patient and family in meeting healthcare-related needs
Requirements:
  • Current license as an LPN in the State of MN
  • 2 years' experience as an LPN
  • Able to travel within the geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
