Pediatric Nurse Care Coordinator

Geisinger is seeking a Pediatric Nurse Care Coordinator to join our team in Danville, PA. As a member of our Enhanced Member Supports Unit, you will be responsible for providing patient-centered care management services to members with special needs.
We are looking for a skilled and compassionate LPN with at least 5 years of experience in pediatric nursing. Your primary goal will be to ensure that all members receive necessary services and supports in a timely manner.
Key Responsibilities:
- Perform onsite and telephonic reviews to evaluate needs and determine appropriate management
- Meet regulatory requirements related to management of Shift Care members, including home support, Durable Medical Equipment, access to primary and specialty care, medication review, childcare access, transportation, financial strain, and housing
- Serve as single point of contact for navigating health system and health plan needs
- Provide culturally competent, evidence-based management to pregnant members
- Partner with Maternal Home Visiting programs to facilitate individualized, strength-based, and family-focused care
- Foster a team approach and serve as liaison between the member, family, treatment team, and community to ensure all needs are being met and Social Determinants of Health are addressed
- Collaborate with school personnel to ensure the child's educational needs are being met
- Escalate and communicate worsening condition or urgent needs to appropriate care team members
- Keep detailed records of all contacts made with the child and family, as well as any other pertinent information
Requirements:
- Minimum of 5 years-Nursing experience
- Licensed Practical Nurse - Default Issuing Body
