About the Role

We are seeking a highly skilled and compassionate Home Health LPN to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide Professional Nursing Services: Under the direction of a RN, you will provide skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care.
  • Develop and Implement Care Plans: Utilize a holistic approach to develop and implement individualized care plans that address patients' unique needs and goals.
  • Communicate Effectively: Maintain open and effective communication with patients, physicians, clinical managers, and other healthcare team members to ensure seamless care coordination.
  • Document Patient Care: Prepare, submit, and maintain accurate and timely documentation of patient care services, including visit/shift notes and other required documentation.
  • Collaborate with Care Team: Participate in care coordination activities and discharge planning to ensure a smooth transition of care for patients.
Requirements
  • Graduate of an Accredited LPN Program: Must be a graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current Nursing License: Must hold a current nursing license in the practicing state.
  • Valid Driver's License: Must possess a valid driver's license, auto insurance, and reliable transportation.
  • CPR Certification: Must hold current CPR certification.
  • Experience in Home Health or Hospice Setting: Must have at least two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
