LPN Home Care Position Summary

We are seeking a skilled LPN to provide exceptional care to patients in their homes. As a Home Care LPN, you will use cutting-edge technology and clinical knowledge to deliver personalized care and support to patients who require one-to-one attention and monitoring.

Responsibilities:
  • Client assessment and care planning
  • Providing one-to-one care and monitoring in patients' homes
  • Following physician orders and adhering to state Nurse Practice Act and licensure/certification requirements
  • Collaborating with interdisciplinary teams to ensure comprehensive patient care
Requirements:
  • Graduate of an accredited or state-approved school of practical nursing
  • Licensure as an LPN in the State of Georgia
  • Minimum one year of experience in an acute care setting; home care experience preferred
  • Phlebotomy and wound care experience required
