LPN Home Care Revisit Job Summary

We are seeking a compassionate and skilled LPN to provide one-to-one care to patients in their homes. As a Home Care LPN, you will be responsible for client assessment, care planning, intervention, and overall supervision of patient care and outcomes.

Key Responsibilities:
  • Provide high-quality, patient-centered care to patients in their homes
  • Assess patient needs and develop individualized care plans
  • Implement and evaluate care plans to ensure optimal patient outcomes
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Document patient information and progress in a timely and accurate manner
Requirements:
  • Graduate of an accredited or state-approved school of practical nursing
  • Licensure as an LPN in the State of Georgia
  • Minimum one year of experience in an acute care setting; home care experience preferred
  • Phlebotomy and wound care experience required
