Role Overview

The Advanced Practice Provider at the Neighborhood Center functions as a healthcare professional who drives, supports, diagnoses, and manages a variety of health-related issues for our patients. Under the supervision of the Neighborhood Center Medical Director, the provider will oversee patients in a multidisciplinary, value-based care model. The role involves managing a range of patients from low-risk to high-complexity cases, requiring thorough evaluation of various factors. The clinician will collaborate with a multidisciplinary care team and lead their respective care team within the Neighborhood Center.

Key Responsibilities
  • Provide care for patients, focusing on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
  • Oversee patient management under the guidance of the Neighborhood Center Medical Director in a multidisciplinary, value-based care model.
  • Lead the respective care team within the Neighborhood Center.
  • Collaborate effectively with the multidisciplinary care team.
  • Strive to deliver 5-Star care and maintain a high level of customer service.
  • Maintain accurate medical histories and records.
  • Refer patients to specialists as necessary.
  • Participate in daily morning huddles with the care team to discuss patients scheduled for the day and address any potential care gaps.
  • Engage in weekly huddles with the multidisciplinary care team to review and manage high-risk patients.
  • Contribute to the department's strategic direction.
  • Make informed decisions on moderately complex to complex issues regarding project components, working independently.
  • Exercise significant discretion in determining objectives and approaches to assignments.
Qualifications

Knowledge, Skills, and Abilities
  • Minimum of 2 years of experience, preferably in geriatrics.
  • Active, unrestricted medical license in Texas or Arizona.
  • Current on all required immunizations.
  • Understanding and support of a managed care, full risk, value-based care model preferred.
  • Strong leadership, customer service, and communication skills, both spoken and written.
  • Ability to demonstrate self-confidence and a positive attitude.
  • Excellent teamwork and communication skills required.
  • Proficiency in Microsoft Office applications including Word, Excel, and PowerPoint.
  • Bilingual/Bicultural (English and Spanish) preferred.
Education and Certification Requirements
  • Nurse Practitioner (NP) or Physician Assistant.
  • Board certification or eligibility in Family, Adult, and Geriatrics.
  • Current DEA license.
