Key Responsibilities

The Clinical Pharmacist plays a crucial role in ensuring the safe and effective use of medications. This includes:

  • Evaluating prescriber orders for accuracy, potential allergies, and contraindications.
  • Supervising pharmacy support staff and addressing pharmacy-related inquiries from healthcare professionals and the public.
  • Conducting pharmacokinetic monitoring and consultations, reporting any adverse drug events, and recommending cost-effective medication options.
  • Providing medication counseling to patients and their families, ensuring they understand their treatment plans.
  • Actively participating in the medication reconciliation process during patient admissions, transfers, and discharges.
  • Engaging in the education of pharmacy students and residents, promoting the profession of pharmacy.
Essential Functions

The Clinical Pharmacist is responsible for:

  • Reviewing medication therapy and collaborating with the healthcare team as necessary.
  • Assessing medication orders for appropriate selection, dosage, and potential interactions.
  • Monitoring patient profiles to ensure ongoing appropriateness of therapy.
  • Utilizing formulary agents effectively and managing the procurement of non-formulary items.
  • Ensuring accurate and timely dispensing of pharmaceuticals, including controlled substances and chemotherapy.
  • Providing supervision and timely checks of products prepared by pharmacy staff.
  • Delivering educational opportunities to healthcare practitioners, including students and residents.
Qualifications

Education:

  • Doctor of Pharmacy or Bachelor's Degree in Pharmacy from an accredited institution.

Experience:

  • Preferred: PGY1 Residency or a minimum of three years in a hospital pharmacy setting.

Licenses and Certifications:

  • Licensed Pharmacist (PHARMD) required.
  • Basic Life Support (BCLS) certification required.
  • Board Certified Pharmacotherapy Specialist (BCPS) preferred.
