Behavioral Health Support Specialist

This role provides behavioral health support to residents of long term care facilities under the supervision of a licensed professional. The ideal candidate will have a strong understanding of behavioral health principles and be able to work effectively in a team environment.
Key Responsibilities- Conduct assessments to identify the behavioral and social needs of patients
- Meet with families and facility staff to coordinate care and develop individualized treatment plans
- Provide education and support to patients and their families to promote self-management and adherence to treatment plans
- Assist in the development of individual goals and behavioral health treatment plans
- Coordinate with other agencies and community providers to ensure comprehensive care
- Maintain timely and accurate documentation of work
- Bachelor's degree in Counseling, Social Work, Psychology, or related field
- Experience in a psychiatric or long term care setting preferred
- Strong customer service skills and ability to work effectively in a team environment
- Effective use of electronic medical records and Microsoft software
- Ability to handle confidential and sensitive information
The work environment is typically low to moderate noise level. The employee must be able to interact effectively with others in an office environment, manage conflict, and handle stressful situations and deadlines. Ability to work flexible hours as needed to meet deadlines.
