Job Summary

This role provides behavioral health support to residents of long term care facilities under the supervision of a licensed professional. The ideal candidate will have a strong understanding of behavioral health principles and be able to work effectively in a team environment.

Key Responsibilities
  • Conduct assessments to identify the behavioral and social needs of patients
  • Meet with families and facility staff to coordinate care and develop individualized treatment plans
  • Provide education and support to patients and their families to promote self-management and adherence to treatment plans

Requirements

  • Effective use of electronic medical records and Microsoft software
  • Ability to handle confidential and sensitive information
