MedStar Health seeks a skilled Nurse Medical Manager to join our Case Management Team. As a key member of our team, you will apply your RN clinical expertise to coordinate care and develop discharge plans that meet individualized patient outcomes.
Key Responsibilities:
- Collaborate with the multidisciplinary healthcare team to develop and coordinate patient care plans.
- Communicate with patients, families, and healthcare teams to address patient goals and issues.
- Evaluate and document patient responses to care plans and make recommendations for modifications as needed.
- Maintain knowledge of regulatory agencies' requirements and reimbursement methods for different levels of care.
- Identify essential resources needed to implement care plans.
Requirements:
- Associate's degree in Nursing (ADN) required; Bachelor's degree in Nursing (BSN) preferred.
- Ability to use computer to enter and retrieve data.
- Working knowledge of Microsoft Word, Excel, and PowerPoint applications.
Salary Range: $87,318 - $157,289
