Key Responsibilities:

  • Collaborate with the multidisciplinary healthcare team to develop and coordinate patient care plans.
  • Communicate with patients, families, and healthcare teams to address patient goals and issues.
  • Evaluate and document patient responses to care plans and make recommendations for modifications as needed.
  • Maintain knowledge of regulatory agencies' requirements and reimbursement methods for different levels of care.
  • Identify essential resources needed to implement care plans.

Requirements:

  • Associate's degree in Nursing (ADN) required; Bachelor's degree in Nursing (BSN) preferred.
  • Ability to use computer to enter and retrieve data.
  • Working knowledge of Microsoft Word, Excel, and PowerPoint applications.

Salary Range: $87,318 - $157,289
