We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at AccentCare. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.
Key Responsibilities- Assess patient needs and develop personalized care plans in collaboration with the patient, family, and healthcare team.
- Provide education and support to patients and caregivers on disease management, medication, and other relevant topics.
- Coordinate care with the Interdisciplinary Group (IDG) and ensure seamless communication with the patient, family, and healthcare team.
- Document patient progress and maintain accurate records in accordance with protocol.
- Associate degree in nursing or higher.
- Current and valid RN license in the state(s) practicing.
- Minimum one year of experience in hospice, home health, or general medical-surgical nursing.
- Previous experience in patient population-specific nursing (e.g., geriatrics, pediatrics) is a plus.
- Willingness to serve on-call and have a reliable automobile, valid driver's license, and minimum state required liability auto insurance.
