Our team provides comprehensive care across the continuum, from home and virtual settings to skilled nursing facilities, assisted living, and acute care hospitals.

We offer a collaborative environment where our physicians and nurse practitioners work together to drive the best patient outcomes. Our team includes intensivists, oncologists, geriatricians, family medicine, emergency medicine, internal medicine, nephrologists, and hospice and palliative care specialists.

Responsibilities

As a Visit Physician, your key responsibilities will include:

  • Providing symptom management and ongoing goals of care discussions with patients and their families.
  • Developing and maintaining excellent communication skills to work effectively with seriously ill and/or terminal patients and their families.
  • Collaborating with the medical director, team physicians, and team leaders to ensure continuity of care consistent with the hospice philosophy and patient wishes.
  • Working closely with all members of the hospice team to provide comprehensive care.
  • Completing all documentation within 24 hours of the visit.
Qualifications

To be successful in this role, you will need to maintain an active License and DEA in the state where care is being provided.
