We are seeking a compassionate and energetic Licensed Clinical Social Worker (LCSW) to join our team at Long Island Select Healthcare Inc. (LISH). As a Federally Qualified Health Center (FQHC), we provide comprehensive community health services at three locations across Suffolk County, New York.
Key Responsibilities- Provide individual, marital, family, and group psychotherapy as referred.
- Conduct psychosocial assessments and mental health intake assessments as needed.
- Maintain close liaison with other care providers, including psychiatrists, to ensure seamless patient care.
- Prepare and update treatment care plans, daily notes, and discharge notes in a timely manner.
- Communicate program information to patients, including brochures and program values.
- Attend in-service education programs to stay up-to-date on best practices.
- Travel between Agency buildings as needed.
- Maintain licensure in NYS by completing 36 hours of continuing education (CE) every three years.
- Master's Degree from an accredited School of Social Work in the United States.
- Minimum 2 years of clinical experience.
- NYS Licensed Clinical Social Worker.
- Basic Life Support Certification.
- Bilingual in Spanish/English preferred.
- Ability to travel to other sites for coverage as needed.
- Eligibility for benefits with a minimum of 30 hours/week.
- Competitive hourly rate starting at $39, negotiable based on experience.
- Eligibility for Student Loan Repayment Programs.
- 4 weeks of paid vacation, 10 sick days, 3 personal days, and 10 holidays per year.
