LPN - Adult Program

Crystal Springs is a non-profit organization that provides educational, therapeutic, and residential services to children and adults living with both physical and intellectual disabilities.

About the Role

We are seeking a skilled and compassionate LPN to join our team as an Adult Program LPN. In this role, you will provide direct nursing support to adults with intellectual and developmental disabilities in a safe, empowering, and satisfying work environment.

Responsibilities
  • Perform bimonthly in-home audits and medication and MAR reviews
  • Maintain health records according to DDS standards, ensuring that necessary documentation is up to date
  • Assess and coordinate healthcare services for individuals, initiating follow-up and communication of all medical concerns to the Director of Nursing, individuals' team, parents/guardians, Area Director, medical providers, and any other vendor/s involved in the individuals' care
  • Maintain current medication and treatment orders, noting any side effects/adverse effects and reporting to the Director of Nursing, individuals' team, parents/guardians, Area Director, medical providers, and any other vendor/s involved in the individuals' care
  • Assist in training staff in medication policies and procedures
  • Perform physical assessments, including vital signs such as blood pressure, temperature, and weight, and provide basic wound care, including cleaning and bandaging injured areas when needed
  • Administer medications when needed/required in adherence to MAP regulations
  • Provide G and J tube care as needed
  • Review ISPs for current knowledge of programming in self-medication, behavior plans, hygiene skills, and physical therapy plans
  • Review all appropriate tracking logs (BM, menses, etc.) and provide additional training/retraining when necessary
  • Order medication from pharmacy as needed
  • Provide MAP training/education to staff pursuing MAP certification
  • Meet with Director of Nursing for supervision and direction as needed to ensure appropriate medical health care is provided for individuals
  • Attend staff meetings to present all current medical/health issues and concerns to the team, as well as upcoming medical appointments
  • Attend all designated mandatory in-services
Qualifications

To implement procedures and services, including medical interventions, or additional duties, as assigned by the Director of Nursing:

  • LPN with current MA license
  • New graduates welcome with at least one year of experience as an LPN is preferred
  • Valid Driver's License
