Job Summary

We are seeking a Senior Healthcare Coordinator to join our team at CAI. As a key member of our healthcare coordination team, you will be responsible for providing exceptional care coordination services to patients with complex health needs in the state of Delaware.

About the Role

In this role, you will travel to patients' homes and facilities to conduct face-to-face assessments, develop personalized care plans, and coordinate services across the continuum of care. You will work closely with healthcare professionals, community resources, and family members to ensure that patients receive the best possible care.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, behavioral, and social needs
  • Develop and implement individualized care plans to address patients' complex health needs
  • Cordinate services with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Educate patients and their families on available benefits, resources, and services to support their health and well-being
  • Provide ongoing case management services to monitor patients' progress and make adjustments to care plans as needed
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse license in the state of Delaware
  • At least 5 years of experience in case management and discharge planning, including experience working with patients with complex health needs
  • Strong communication and interpersonal skills to effectively collaborate with healthcare teams and patients
  • Ability to work independently and as part of a team to achieve patient-centered outcomes
