As a key member of our interdisciplinary team, you will be responsible for providing comprehensive social work services to our residents, ensuring their physical, emotional, and social needs are met.

Key Responsibilities:
  • Process inquiries and admissions, ensuring a smooth transition for new residents.
  • Coordinate marketing and community relations efforts to promote our services and build relationships with local agencies.
  • Participate in multidisciplinary quality and service improvement teams, focusing on resident care, goals, and approaches.
  • Maintain cultural competency and provide leadership in cultural competence for the facility.
  • Develop and maintain a community resource file, providing information and referrals to residents and their families.
  • Develop relationships with families and associates, incorporating them in care planning and fostering independence.
  • Develop and maintain therapeutic group activities, such as family support groups, and develop cultural competence with each resident and their family.
  • Update the care plan as needed, ensuring resident needs are met and documented.
  • Participate in interdisciplinary care conferences to review, develop, and revise the plan of care.
  • Represent the department and nursing center in community outreach efforts, attending conferences, seminars, and workshops to stay up-to-date on best practices.
Requirements:
  • Bachelor's degree in social work or related field.
  • At least 2 years of related long-term-care experience.
