Job Summary: Liberty Healthcare and Rehabilitation Services seeks a motivated Healthcare Professional - Rehabilitation to join our team. The ideal candidate will have experience working in rehab SNF settings and be licensed in the state where the SNF is located.

About the Role:
In this role, you will work as part of a multidisciplinary team to provide exceptional patient care. Your expertise will be utilized to develop and implement treatment plans that address the unique needs and goals of each patient. You will also have the opportunity to work in a supportive rehabilitation environment, utilizing progressive treatment modalities to address a variety of diseases and impairments.

Key Responsibilities:
  • Develop and implement individualized treatment plans for patients
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Utilize progressive treatment modalities to address a variety of diseases and impairments

Requirements:
Must be a graduate of an accredited Therapy program and licensed in the state where the SNF is located. Previous experience in rehab SNF settings is preferred.

Salary Range: $65,000 - $90,000 per year.
