About the Role
We are seeking a highly skilled and compassionate Transplant Coordinator II to join our kidney-pancreas transplant team at Penn Medicine. In this role, you will be responsible for providing direct care and coordinating care for patients with end-stage organ failure referred to the Transplant Program. You will work closely with our multidisciplinary team to ensure that patients receive seamless care throughout the transplant process, from referral to post-transplant care.

As a Transplant Coordinator II, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will assess and evaluate patients and families, developing a care plan in collaboration with the transplant team. You will also create a written, continuous record for each patient, implement identified medical and nursing interventions, and evaluate outcomes.

We offer a competitive salary range of $83,000 - $103,000 per year, commensurate with experience.
