**Job Title:** Transplant Clinical Pharmacist Specialist

**Estimated Salary:** $120,000 - $160,000 per year (based on location and industry standards)

**Overview:** Thomas Jefferson University is a nationally ranked healthcare institution seeking an experienced Transplant Clinical Pharmacist to join our team at the Jefferson Einstein Philadelphia Hospital. As a key member of our multidisciplinary transplant team, you will provide comprehensive pharmacy services to abdominal solid organ transplant candidates, recipients, and living donors.

**Responsibilities:*** Works as part of the transplant team to ensure optimal drug therapy and monitoring of transplant recipients and living donors on the inpatient transplant service* Participates in multidisciplinary rounds and ensures thorough medication reconciliation* Provides discharge planning and patient education, following up with post-transplant recipients in the multidisciplinary transplant clinic* Assists in the development/updating of existing practices, protocols, guidelines, and order sets* Serves as a drug information and educational resource for the pharmacy staff, medical staff, nursing staff, and other healthcare professionals* Precepts PGY1 Pharmacy residents and P4 Pharmacy students* Opportunities to lead and participate in research and quality improvement projects* Maintains education in transplant pharmacotherapy advances, guidelines, and literature to provide evidence-based care and contribute to the professional development of the transplant team.

**Requirements:** Graduate of ACPE accredited college of pharmacy (BS or PharmD) Licensure to practice Pharmacy in the Commonwealth of Pennsylvania PGY1 residency program completion or equivalent experience in pharmacy Preferred qualifications: PGY2 Transplant Pharmacy residency completion or equivalent experience in transplant pharmacy Board certification in Solid Organ Transplantation Pharmacy (BCTXP) or board certification-eligible within 1 year of employment
