Job Overview

We are seeking a dedicated Home Health Assistant to join our team at The Ridge at Grandview. This is a full-time opportunity that requires strong communication and interpersonal skills, as well as the ability to work independently.

Responsibilities:
  • Provide personal care and support to residents in accordance with their individualized service plans.
  • Document resident information and services provided in a timely and accurate manner.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in meal service and maintain a clean and safe environment for residents.
  • Foster a positive and respectful team culture by promoting open communication and collaboration.
Requirements:
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
  • 1 year of experience in a caregiving role or similar function.
  • Excellent verbal and written communication skills.
  • Ability to work flexible hours, including evenings and weekends.
