**Job Summary:**
Performs patient evaluations and re-evaluations, establishes patient treatment plan based upon the referral from the physician/referral source. Documents the patients' care and directs supportive staff regarding treatment process. Provides leadership to Certified Occupational Therapy Assistants, Students, Rehab Aides and Volunteers.

These duties are performed in a manner that demonstrates the philosophy/mission and values of Mercy Health.

**Key Responsibilities:**

* Reviews physician referral, patient's condition and medical history to determine occupational therapy treatment needs.
* Performs and documents patient evaluation, re-evaluation and develops treatment and discharge plans.
* Plans and administers medically prescribed occupational therapy treatments to patients.
* Designs and constructs special equipment for adaptation of individual's work and living environment.
* Plans programs involving activities, practice in functional, pre-vocational, vocational, homemaking skills, activities of daily living and sensory motor activities.
* Modifies treatment goals and plan of care as appropriate. Involved with patient care conferences as appropriate.
* Instructs patient and family in treatment procedures to be continued at home.
* Responsible for overall management of patient cases including communication and coordination of care with treatment team.
* Precepts in the direction and evaluation of clinical experience / internship of students from participating colleges or universities.
* Orients, instructs and directs the work activities of assistants, aides, students and volunteers.
* Stays abreast of current developments and new technology in occupational therapy.
* Demonstrates behaviors consistent with Mission and Core Values. Adheres to standards and policies, laws and regulations. Demonstrates the ABC's of Mercy Health Culture. Holds self and others accountable.

**Requirements:**

* Required Minimum Education: 4 year/ Bachelors Degree in Occupational Therapy
* Licensure/Certification Required: Licensed in State of Ohio PT/OT/AT Board
* Licensure/Certification Preferred: CPR Certification
