Eden Senior Care, a leading healthcare management company, is seeking a skilled Licensed Practical Nurse to join our team at Edenbrook of Fond Du Lac, a skilled nursing facility in Fond du Lac, Wisconsin.

Job Summary

We are looking for a dedicated and compassionate LPN to provide high-quality patient care and support our team in delivering exceptional service to our residents and their families.

Responsibilities
  • Provide direct and indirect patient care through assessment, planning, prevention, and evaluation.
  • Direct the day-to-day activities of nursing assistants and prepare and administer medications as ordered by physicians.
  • Communicate effectively with the healthcare team, residents, and families to ensure seamless care coordination.
  • Complete required record-keeping and charting according to facility policies.
Requirements
  • Graduate of an accredited nursing program and current state licensure as a Licensed Practical Nurse.
  • Minimum of 1 year of experience in a long-term care setting, with a strong focus on customer service and teamwork.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Able to lift, push, and pull up to 25 pounds, with frequent standing, walking, and bending.
