House Calls Emergency Medicine Physician Assistant or Nurse Practitioner

As an Advanced Care Provider, you will conduct concierge medical house calls to Sollis Health patients, treating them under the supervision of an attending physician. This role is pivotal in providing seamless care to adult and pediatric patients for cases that would typically require an emergency room or clinic visit. You will provide various treatments and care during house calls, reducing the need for patients to visit the emergency department and receiving care directly in their homes.
Responsibilities- Formulate a differential diagnosis and treatment plan for urgent and emergent care in emergency medicine settings.
- Coordinate with House Call Coordinators for requests and respond to patient care requests in a timely manner.
- Perform medical evaluations and use imaging studies as part of patient assessments.
- Ensure the house call provider bag is properly stocked at all times to avoid delays.
- Conduct Covid-related house calls, obtaining clinical samples, performing point of care testing, and providing general symptom management and recommendations.
- Perform medical procedures such as obtaining IV access, splinting, suturing, phlebotomy, and EKGs.
- Document clinical evaluations and treatment in the electronic health record.
- Maintain patient comfort, address patient questions, and use patient-centered communication.
- Interface with patient PCPs and specialists to discuss assessment and plan of care.
- Work closely with Medical Directors and clinical team members to ensure policies are adhered to and quality care is delivered.
We seek candidates with a Bachelor's Degree, Physician Assistant or Nurse Practitioner state license, and ability to practice in New York State. Preferred candidates have 2-3 years of experience in emergency departments and/or urgent care centers, along with DEA licensure and BLS, ACLS, PALS certification.
SkillsCandidates will demonstrate strong clinical skills, technical savvy, and ability to use our systems. They will also possess superb technical skills, including phlebotomy, IV placement, and medication administration. Adaptability, problem-solving, and organizational skills are essential, as well as the ability to interface with five-star clientele.
