Role Responsibilities:
  • Collaborate with the interdisciplinary team to assess, plan, coordinate, and evaluate resident care.
  • Maintain accurate and timely documentation of resident conditions and nursing requirements.
  • Communicate significant observations and changes in resident status promptly.
  • Supervise and guide nursing assistant assignments, adjusting as necessary based on resident needs.
  • Administer treatments and document responses to medications and interventions.
  • Prepare, administer, and record all medications accurately.
  • Facilitate communication with families regarding medication changes and resident updates.
  • Ensure care plans are regularly reviewed and reflect the unique needs of each resident.
  • Coordinate admissions, discharges, and transfers to enhance service quality.
Qualifications:
To be considered for this role, candidates must possess:
  • A degree in Practical Nursing from an accredited institution.
  • Current Licensed Practical Nurse license and any additional certifications required by state regulations.
