Role Responsibilities:- Collaborate with the interdisciplinary team to assess, plan, coordinate, and evaluate resident care.
- Maintain accurate and timely documentation of resident conditions and nursing requirements.
- Communicate significant observations and changes in resident status promptly.
- Supervise and guide nursing assistant assignments, adjusting as necessary based on resident needs.
- Administer treatments and document responses to medications and interventions.
- Prepare, administer, and record all medications accurately.
- Facilitate communication with families regarding medication changes and resident updates.
- Ensure care plans are regularly reviewed and reflect the unique needs of each resident.
- Coordinate admissions, discharges, and transfers to enhance service quality.
To be considered for this role, candidates must possess:
- A degree in Practical Nursing from an accredited institution.
- Current Licensed Practical Nurse license and any additional certifications required by state regulations.
