About the Role

We are seeking a dedicated and compassionate Housing Stabilization Specialist to join our team at MySkye Care. This role will involve providing community-based housing stabilization services to eligible adult clients with disabilities, empowering them to live successfully in their communities.

Key Responsibilities:

  • Develop and implement person-centered housing plans for clients, ensuring their unique needs are addressed.
  • Maintain accurate records of client interactions, progress, and goals, utilizing software applications like Microsoft Office Suite.
  • Collaborate with clients, families, and healthcare professionals to ensure comprehensive support and advocacy.
  • Conduct home visits and provide in-home support services as needed, promoting independence and self-sufficiency.
  • Participate in training and professional development opportunities to enhance knowledge and skills in areas such as crisis intervention, cultural competency, and trauma-informed care.
  • Work closely with internal teams, including supervision, under the direction of designated supervisors, to achieve program objectives and maintain high-quality services.
  • Perform other duties as assigned, contributing to the overall success of MySkye Care.

Requirements:

  • Bachelor's degree in social sciences, e.g., Social Work, Psychology, Human Services, Chemical Dependency, Sociology.
  • At least one year of experience working in a community setting providing similar services, with a strong focus on case management and client advocacy.
  • Valid Minnesota driver's license, reliable transportation, and valid insurance for work-related travel.

Salary: $60,000 - $80,000 per annum, depending on experience and qualifications. Benefits include employee discount, flexible schedule, paid time off, professional development assistance, referral program, 8-hour shift, Monday to Friday, no weekends, self-determined schedule, high school or equivalent (required), case management: 1 year (required), driver's license (required), car insurance (required), and willingness to travel 50% (required).
