Key Responsibilities- Perform related work as required.
- Participate in pandemic response, including mass vaccination clinics.
- Provide 24/7 response to Emergency/Bioterrorism events.
- Conduct telephonic and in-home comprehensive client assessments for individuals aged 60 and over.
- Perform case management duties, including assessment, monitoring, and communication with vendors.
- Establish and maintain relationships with Aging Network organizations.
- Perform patient assessment and examinations, administer immunizations, and maintain medical inventory and supplies.
- Provide education and training on sexually transmitted disease prevention and treatment.
- Investigate communicable diseases, maintain related documentation, and develop effective planning to increase reporting and decrease transmission.
- Assist with community outbreak investigations and maintain effective communication with MDHHS.
