We are seeking an Advanced Primary Care Practitioner to join our team in Englewood, NJ. As a key member of our clinical team, you will play a vital role in providing exceptional patient care and contributing to our mission of revolutionizing healthcare delivery.
About the Role- This is a full-time position that requires a Family Nurse Practitioner or Physician Assistant with national certification and at least 5 years of experience in outpatient primary care or a similar setting.
- You will manage a diverse patient panel, including acute, chronic, and well visits, and treat patients in-office, testing centers, or via tele-health.
- Collaboration with in-office and virtual clinical teams is essential, as is participation in weekly Clinical Rounds for continuous learning and professional development.
The ideal candidate will have a passion for human-centered primary care, effective communication skills, and proficiency in evidence-based primary care practices.
Requirements- Completed FNP or PA program with national certification.
- At least 5 years of Advanced Practitioner experience in outpatient primary care or a similar setting.
- State-licensed in New Jersey by start date.
