We are seeking a skilled Clinical Care Specialist to join our team at Country Meadows. As an Assisted Living LPN, you will have the opportunity to make a meaningful impact on the lives of our residents and their families.

**Responsibilities**

* Form strong relationships with residents, their loved ones, and physicians to create personalized care plans
* Coach and support co-workers to ensure seamless, quality care is delivered around the clock
* Educate residents and their families about health and wellness issues relevant to their care plans
* Serve as a leader and share responsibilities with the Manager on Duty

**Requirements**

* LPN license in the state where employed
* CPR certification (we will pay for certification if needed)
* Minimum of one year experience in assisted living or other healthcare setting
* Strong leadership skills and ability to form relationships with residents, their loved ones, and co-workers
* Ability to perform physical tasks required in this role
