Job Summary

Helio Health is seeking a highly skilled and compassionate Discharge Planner to join our team. As a key member of our Forensic Evaluation Unit, you will play a vital role in supporting the recovery of individuals struggling with substance use and mental health disorders.

Key Responsibilities
  • Maintain a caseload of patients seeking treatment for substance use and mental health disorders.
  • Perform essential counseling responsibilities, including screening, intake, and orientation, assessment, evaluation, and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting, and record-keeping.
  • Participate in program, administrative, and clinical committees as assigned.
  • Relate effectively with staff, clients, and community while upholding confidentiality requirements.
Requirements
  • High School Diploma/GED required and demonstrated steady progress towards QHP status.
  • Associate's or Bachelor's degree in Human Services with CASAC-T preferred.
  • One year of counseling/human services experience preferred.
  • Equivalent alternative combination of education and experience may be substituted for the educational requirements.
  • Knowledge of addiction/recovery, behavioral health, self-help group process, and principles of counseling.
  • Proficiency with basic computer applications.
