Job Summary

We are seeking a skilled Discharge Planner to join our team at La Frontera Center Inc. As a key member of our behavioral health team, you will be responsible for coordinating discharge plans and providing case management services for adults designated Seriously Mentally Ill.

Key Responsibilities
  • Assist the treatment team in formulating discharge plans and service planning for all clients in inpatient settings.
  • Participate in hospital staffing and treatment team meetings to assist in inpatient discharge planning.
  • Work with the Court Coordinator to ensure paperwork is completed accurately and timely.
  • Collaborate with the outpatient team to ensure smooth transitions for clients.
  • Develop and maintain relationships with community resources and service providers.
Requirements
  • Bachelor's Degree in a behavioral health field or a related field.
  • Minimum of 2 years of experience in case management or inpatient settings.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Detail-oriented and organized with excellent time management skills.
