Therapeutic Care Specialist

We are seeking a skilled Physical Therapist to join our home health team. Our goal is to provide high-quality patient care and improve their quality of life. The ideal candidate will have experience in physical therapy and excellent communication skills.
Key Responsibilities:
- Collaborate with other healthcare professionals to achieve well-rounded care.
- Develop and implement individualized physical therapy plans in accordance with physician orders.
- Assess and document patient progress, making necessary revisions to their care plan.
- Supervise physical therapy assistants as assigned and maintain accurate records.
- Ensure timely and accurate documentation of patient visits and progress.
Requirements:
- Current licensure as a physical therapist in the state.
- Minimum of 6 months of experience as a physical therapist.
