LPN - Home Health Care

About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health Care team in Salisbury, NC. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver individualized care plans.

Responsibilities

  • Provide 1:1 care for clients, following their plan of care and maintaining their dignity and independence.
  • Monitor clients' vital statistics and report abnormal findings to the Case Manager and Physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, per client's plan of care and provided assessments of results.
  • Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
  • Ensure infection control policies are maintained at all times, reporting possible communicable diseases or infections and implementing infection control procedures.
  • Accurately document nursing actions, communication with family, case manager, and physicians, and documenting progress and outcomes for established goals.
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
  • Ensure all patient needs are being met in accordance with the instructions of the physician.
  • Monitor incidents involving individuals, ensuring incident reports are completed timely and accurately, and informing case managers.

Requirements

  • LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
  • Able to lift and carry items weighing up to 20 pounds.
  • Able to bend, stoop, push, pull, sit, and walk for periods of time.
  • Must be able to communicate both verbally and in writing.
