We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Adoration Home Health and Hospice. As a Home Health LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.
Responsibilities- Provide 1:1 care for clients, following their individualized care plans and maintaining their dignity and independence.
- Monitor clients' vital statistics and report any abnormal findings to the Case Manager and Physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and assess the results.
- Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
- Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
- Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
- LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications.
- 1+ years of nursing experience preferred.
- Home health care experience preferred.
