Responsibilities:- Collaborate with physicians, social workers, chaplains, and other healthcare professionals to develop and implement patient care plans.
- Assess patients' medical conditions and provide guidance to loved ones on end-of-life care.
- Administer pain management medications and perform prescribed treatments as needed.
- Provide emotional support and education to patients and their families on coping techniques and available resources.
- Maintain accurate and up-to-date medical records and charts.
- Bachelor of Science degree in nursing (BSN) and state-approved licensure as a registered nurse (RN).
- Accredited certification in hospice and palliative care is preferred.
- Minimum of 2 years of experience as a hospice nurse, with competency in electronic patient and hospice management systems.
The physical demands of this position require the ability to stand, walk, sit, and reach, with occasional lifting of up to 50 pounds. The work environment is typically low to moderate noise level.
