About the Agency on Aging of South Central CT

We are seeking a highly motivated and compassionate Care Manager to join our team in North Haven, CT.

As a Care Manager, you will be responsible for ensuring the well-being of our older adult clients in a home care setting.

Your major end result will be to provide exceptional social services to our clients, including discharge planning, mental health support, and long-term care management.

You will work closely with our clients and their families to develop personalized care plans that meet their unique needs and preferences.

The salary range for this position is $47,500 per year.

Job Responsibilities:

  • Develop and implement personalized care plans for our clients in a home care setting.
  • Provide exceptional social services to our clients, including discharge planning, mental health support, and long-term care management.
  • Collaborate with clients and their families to ensure that their needs and preferences are met.
  • Maintain accurate and up-to-date client records and documentation.
  • Communicate regularly with healthcare providers and other members of the care team to ensure continuity of care.

Requirements:

  • Bachelor's degree in Human Services or related field.
  • 2+ years of experience in case management or related field.
  • Knowledge of Medicaid regulations and requirements.
  • Excellent communication and interpersonal skills.
  • Patience and compassion for older adults.
