Job Summary

The Home Health Therapist plays a crucial role in assessing and evaluating patient care needs, determining personalized treatment plans, and implementing interventions to enhance patient wellbeing. As an independent contractor, you will be responsible for providing high-quality services defined by state laws governing occupational therapy, collaborating with the healthcare team, and maintaining accurate records of patient progress.

Responsibilities

  • Conduct comprehensive assessments and evaluations of patient care needs, including functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
  • Develop and implement tailored treatment plans, incorporating therapeutic, creative, and self-care activities to improve patient functioning and safety.
  • Monitor and record patient response to treatment, reporting changes to the patient's condition to the healthcare team.
  • Provide education and training to patients, families, caregivers, and the healthcare team on therapy techniques and strategies.

Requirements

  • Demonstrated ability to work with patients of all ages, including adults and children.
  • OCCUPATIONAL THERAPIST LICENSE AND REGISTRATION REQUIRED BY THE STATE.
  • Completion of an accredited Occupational Therapist program.
  • Bilingual skills are a plus, but not required.
