Occupational Therapist Role at Neogen Care

We are seeking a skilled Home Health Occupational Therapist to join our team in Thousand Oaks, CA. As an integral part of our organization, you will work closely with patients, caregivers, and healthcare professionals to deliver exceptional care.

Key Responsibilities:

  • Conduct thorough assessments of patients' physical and psychological needs, developing tailored treatment plans to address their challenges.
  • Evaluate patients' living and working environments, identifying necessary adjustments to promote independence and safety.
  • Recommend adaptive equipment to facilitate daily activities and enhance patient mobility.
  • Create personalized physical rehabilitation programs to help patients regain lost skills and abilities.
  • Prepare patients for a successful return to work by addressing specific needs and goals.
  • Provide education and support to caregivers and family members on patient care techniques and strategies.
  • Maintain accurate records and progress reports, ensuring seamless communication among healthcare teams.
  • Stay updated on the latest treatment practices and technologies, applying this knowledge to deliver high-quality patient care.
  • Ensure compliance with federal, state, and local regulations governing home health care services.

Requirements:

  • A Bachelor's degree in Occupational Therapy from an ACOTE- or AOTA-accredited institution.
  • Certification by the National Board for Certification in Occupational Therapy (NBCOT).
  • Valid California Occupational Therapy license.
  • Current CPR/BLS certification.
  • A minimum of 1 year of recent experience in occupational therapy, preferably in a home health setting.
  • Strong observational, communication, problem-solving, and basic math skills.
  • Familiarity with Electronic Medical Record software (Kinnser) is a plus.
