About the Role

We are seeking a skilled LVN to join our hospice care team. As a vocational nurse, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct thorough assessments and document patient information accurately and timely.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Graduate of a vocational nursing program.
  • California Licensed Vocational Nurse upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Phlebotomy/IV Certification required within 6 months of hire.
  • 1 year of nursing experience.
Preferred Qualifications
  • Community health/hospice/palliative care or medical/surgical experience.
