We are seeking a skilled Registered Nurse to join our team at Providence PACE, a Program of All-Inclusive Care for the Elderly. As a per diem/on call RN, you will provide care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.
Key Responsibilities- Provide care management and care coordination services to patients with complex medical and psychosocial issues.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure comprehensive care and services are provided to patients.
- Travel between multiple locations to provide care and services to patients.
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
