About the Role

Sangre de Cristo Community Care is seeking a Part Time Certified Nursing Assistant to join our dynamic hospice team in Pueblo, Colorado.

Job Summary

The CNA will provide patient care as directed by the RN or Case Manager, actively participating as a member of the interdisciplinary team to assist in the development and implementation of the patient's Plan of Care.

Responsibilities
  • Provide a full range of personal care through regular visits in patients' homes and institutions in accordance with hospice standards and scope of practice as defined by regulatory agencies.
  • Assist with personal hygiene, ambulation, and exercise, and provide nutritional support for patients, respecting personal and professional boundaries.
  • Comply with established Plan of Care and care plans.

  • Competitive Salary - Minimum hourly wage of $17.00 per hour with Weekend Shift Differential.

Requirements
  • Must have a current and clear CNA license in the state of Colorado.
  • At least 1 year experience as a CNA. Home Health or Hospice experience preferred.
  • Must have a clear and valid Colorado Driver's License.
  • Ability to work a flexible schedule to include weekends and holidays.
